Registering as a New Patient
High Street Medical Centre has an "open list" and is accepting new NHS patients. In order to register with the practice you should ensure you live within the practice boundary.

If you are at all unsure as to whether your home address lies in our area, please ask a member of our reception team who will be happy to check for you.
If you would like to proceed with an application to register with the practice, then there are a few simple forms you will need to complete. You can either visit the practice to collect a pack containing all the necessary information, or alternatively print the forms below:
Registration Form (GMS1) - This is required for all new patients.
New Patient Questionnaire (age 16+)
New Patient Questionnaire (under 16s)
You should bring your completed registration form and questionnaire along with your proof of identity (e.g. a passport or photocard driving licence) to the practice in order to finalise your registration.
Once all the necessary information has been received, you will be registered with the practice within 2 working days. If you need an urgent appointment, please explain this to the receptionist, who will be happy to arrange this for you.
